Role Overview

Extended Care LLC residential placement also known as Host Home, provides an inviting family style support environment for individuals with intellectual and developmental disabilities. Extended Care LLC matches people seeking a new home environment with an individual, couple or family in the community who wish to share their life experience with a person with a disability. • The individual who is invited in your home has their own room, is included in family activities both in the home and community. • All initial required training and on-going trainings are provided by Extended Care LLC and are available to complete on-line. • Host Home Providers are independent contractors who receive a daily tax-exempt stipend, plus room and board.

Requirements: • Safe, stable, and caring home environment • Available bedroom in your home. Bedroom has a lock for privacy. • Passion for working with individuals with Intellectual and Developmental Disabilities • Excellent time management, communication, and written/computer skills • Pass Background, Motor Vehicle • Valid driver’s license • Reliable vehicle to transport individual • CPR/First Aid Certified • Qualified Medication Administration Personnel (QMAP) • Auto, Home and Professional Liability Insurance • Annual and quarterly trainings
