Role Overview

Host Home, provides a family like support environment for people with intellectual and developmental disabilities. We match people seeking a new home environment with an individual, couple or family in the community who wish to share their life experience with a person with a disability. Each host home is selected to specifically meet the unique needs of an individual. Each individual will be supported while respecting their personality, capabilities and goals for the future. Host Home Providers are carefully screened and selected. An extensive matching process facilitates finding a home and family that is a great fit for each other. The person who joins you has their own room, is included in family activities and benefits from the host family's connections to the community. All initial required training and on-going trainings are provided by Extended Care and are available on line using Relias. Adult Host Home Providers are independent contractors.

Qualifications: High School Diploma or Equivalent Experience with Developmental Disabilities is preferred but not required Professionalism Focus on individualized care and support Ability to teach others Good written and verbal communication skills Valid driver's license and clean motor vehicle record Clear background screening and reference checks Ability to chart individual progress through a web based system
